    The issue of physician choice and access to care for Medicare recipients arises time and again as Congress has taken last minute action to prevent drastic cuts to physician reimbursements. Only repeated, last-minute actions have saved doctors from substantial pay cuts. Cutting reimbursements for doctors has surface appeal because it does not require seniors to pay additional dollars out-of-pocket. However, there is a hidden cost. Physicians who live under constant fear of substantial cuts may opt to stop serving Medicare patients, resulting in loss of access to care for many seniors. .With no increase in Social Security benefits over the past two years, seniors participating in TSCL surveys say they are putting off visits to the doctors, and many are not filling prescriptions.     The further we read into the Affordable Care Act, the more we learn it doesn't address the issues of rising costs and access to quality care. A majority of my patients were either Medicare beneficiaries or Medicaid recipients. His first hearing as chairman was titled "Drug Pricing in AmericA Prescription for Change" and, in his opening statement, he vowed to get to the bottom of the growing problem. Following that hearing in January, Chairman Grassley called upon seven of the most profitable pharmaceutical companies to testify before the committee, saying: "Patients and taxpayers deserve to hear from leaders in the industry about what's behind this unsustainable trend." .In the meantime, the chairman of the influential Senate Finance Committee Chuck Grassley (R-Iowa) will re-introduce a drug pricing package (S. 2543) he assembled with the ranking member of his committee, Ron Wyden (D-Ore.), and soon call on Senate leaders to allow debate on the measure, a Grassley spokesman announced last week. .According to the president and CEO of the Federation of American Hospitals, "The bill throws a lifeline to caregivers by continuing the pause in the 2% Medicare sequester, and making adjustments to buffer the impact of an ill-conceived change in physician payments during a pandemic." .Elder abuse affects an estimated 14.1 percent of all non-institutionalized older adults each year, and for every case reported, another twenty-three cases go unnoticed.
