Is there any assistance you may lend to resolve this issue? .After spending four days at a local hospital being treated for a broken elbow and pelvis, 93-year-old Lois Frarie went to a nearby nursing home to build up her strength. Her family was later shocked to learn they would have to pay thousands of dollars out-of-pocket since two of the days she spent in the hospital were considered "observation care" as an outpatient. Because she wasn't an inpatient for at least three consecutive days, she didn't qualify for Medicare's nursing home coverage. .Analysts say that deeper Medicare cuts may be back on the table by the end of the year. One of the most controversial proposals would convert Medicare into a system called "premium support." Under the proposal, the government would allocate a fixed amount of money for Medicare beneficiaries' premiums; and people would then purchase coverage from private health plans that would provide all their care. .Currently, when hold harmless is triggered on a nationwide scale there is no provision of law to finance the unpaid portion of Medicare Part B premium increases. Instead, the entire burden of Part B costs is spread over a much smaller number of individuals, which is the 30 percent of Medicare Part B enrollees who are not protected by the hold harmless provision. This has led to significant spikes in Medicare Part B premiums during the hold harmless years, and in 2018 when a COLA finally became payable, to steep jumps in premiums for those whose Medicare Part B premiums were held lower in 2016 and 2017. .The CBO also notes that increasing the payroll tax rate or subjecting more earnings to the payroll tax could improve Social Security's financing. .Medicare is structured to pay more to providers based on how sick people are — not on making them well.
